Though extensive research was performed by OCS, while some articles mentioned the issue and possible cause, none were found to provide the solution at the time. The solution was generated by On-Site Computer Services, Bellingham, WA, USA (www.On-SiteComputer.com).
In some of the more recent versions of Outlook (2013-2016 plus) the "app bar" (contains links to change the folder bar between Mail, Contacts, Tasks, Calendar Folders, et cetera) may move to the far left in a thing vertical bar with the Folder or other bar just to its left. However, this setup option seems to lose function such as the ability to change the order of the apps to select. The "Normal" or default configuration has the app below the Folder or whatever bar.
As of 2023-02-16, the cause was unknown. One article, back then, mentioned it may have come with an update but nothing definitive was offered.
